Have you ever been diagnosed with a psychiatric condition, including but not limited to: Bipolar disorder, Schizophrenia, PTSD (Post-Traumatic Stress Disorder), panic attacks

What, if anything, would stop you from fully participating in all aspects of this training — including the retreat, online trainings, and community practice? (Please be honest — e.g. work, family, mental or physical health, other commitments)

Do you have any of these contraindications of breathwork, currently or in the past 5 years? Cardiovascular issues (e.g. high blood pressure, heart disease), Respiratory conditions (e.g. asthma, COPD), Neurological conditions (e.g. epilepsy, history of seizures), Psychiatric conditions (e.g. bipolar, schizophrenia, PTSD). Recent surgery or physical injuries, Pregnancy, Glaucoma or retinal detachment

This is a demanding and transformational journey. Do you have a support system (friends, whānau, therapist, etc.) you can lean on during this process?
